Vintage 90s Polo Sportsmen navy button down shirt.  This one could pass for a plain navy work shirt but its far from that.  On the left sleeve is the colorful Polo Sportsmen Respect Wildlife Sportsclub patch.  Another nice touch is the green zipper above the left pocket.  Fits true to a medium.
